In consideration of the Alpena Events Complex, known as APlex and DKG Golf, LLC, known as Hubbard Lake Golf Shop from here forward, permitting me to participate in the Sunrise Side Golf and Craft Show, I hereby assume all risk and liability arising out of my involvement in said show. I also agree to hold harmless and indemnify the APlex and DKG Golf, LLC, their agents, officers, employees and families from all liability and responsibility whatsoever. Including but not limited to injury or death and for damage to any property or the property of others, arising out of or resulting from my participation in said show. I further remise, release, and forever discharge the APlex and DKG Golf, LLC, its agents, officers, employees and families from any and all claims, demands, damage and liabilities resulting from my participation in the Sunrise Side Golf and Craft Show. This form is non-transferable. No refunds 30 days prior to the event.
